Domperidone does not cross the blood–brain barrier and produce adverse central antidopaminergic effects, but blocks opioid emetic action in the chemoreceptor trigger zone. Opioids act by binding to opioid receptors, which are found principally in the central and peripheral nervous system and the gastrointestinal tract. These receptors mediate both the psychoactive and the somatic effects of opioids. No matter what type of opioid is used or how it enters the body, usually, higher doses of opioids are more likely to lead to better pain control.

Respiratory depression

Opioid medicine can slow or stop your breathing, and death may occur, especially if you drink alcohol or use other drugs that cause drowsiness or slow breathing. A person caring for you should give naloxone and/or seek emergency medical attention if you have slow breathing with long pauses, blue colored lips, or if you are hard to wake up. Most reviews conclude that opioids produce minimal impairment of human performance on tests of sensory, motor, or attentional abilities. However, recent studies have been able to show some impairments caused by morphine, which is not surprising, given that morphine is a central nervous system depressant.

Precursor to other opioids

Tramadol (Conzip, Qdolo, Ultram), a common pain medication, has been linked with drug abuse and misuse, dependence, and even fatal overdoses. To address these concerns, in 2014 the DEA placed all forms of tramadol into schedule IV of the Controlled Substances Act (CSA). In 2020, more than 56,000 people died from overdoses in the U.S. involving synthetic opioids, including illicitly manufactured fentanyl. More recent data is shocking – in 2022 over 110,000 people were reported to have died from an overdose, of which almost 69,000 were due to synthetic opioids. The extraction is performed in many steps (one amount of crushed plant is extracted at least six to ten times, so practically every alkaloid goes into the solution). From the solution obtained at the last extraction step, the alkaloids are precipitated by either ammonium hydroxide or sodium carbonate. The last step is purifying and separating morphine from other opium alkaloids. In May 2018, the FDA approved Lucemyra (lofexidine hydrochloride) to reduce the severity of withdrawal symptoms (such as anxiety, nausea, stomach cramps, and sleep problems) and allow a more rapid discontinuation of opioids in adults.

Other Medical Problems

The Drug Enforcement Agency was created in 1973, and President Nixon declared ‘The War on Drugs’ noting the high incidence of heroin addiction in the U.S. Plain allosteric modulators do not belong to the opioids, instead they are classified as opioidergics. Your doctor may recommend you get naloxone (a medicine to reverse an opioid overdose) and keep it with you at all times. A person caring for you can give the naloxone if you stop breathing or don’t wake up. Your caregiver must still get emergency medical help and may need to perform CPR (cardiopulmonary resuscitation) on you while waiting for help to arrive. Lucemyra is an oral, selective alpha 2-adrenergic receptor agonist that reduces the release of norepinephrine thought to contribute to opiate withdrawal symptoms. It is NOT a treatment for opioid use disorder (OUD) and is only approved for treatment for up to 14 days.

Narcotics like morphine may cause many different types of side effects, but all cause drowsiness, sedation, and can lead to respiratory depression (difficulty breathing), especially when combined with alcohol or other CNS depressant drugs (which can be fatal). Sometimes a painkiller drug may be referred to as a “narcotic-like” medicine, but any true narcotic medication will have action at one of these opioid receptors. Itching tends not to be a severe problem when opioids are used for pain relief, but antihistamines are useful for counteracting itching when it occurs.

Check with your doctor right away if your baby has an abnormal sleep pattern, diarrhea, a high-pitched cry, irritability, shakiness or tremors, weight loss, vomiting, or fails to gain weight. Tell your doctor right away if you think you are pregnant or if you plan to become pregnant while using this medicine. Check with your doctor right away if you have darkening of the skin, diarrhea, dizziness, fainting, loss of appetite, mental depression, nausea, skin rash, unusual tiredness or weakness, or vomiting. If a nursing mother is an ultra-rapid metabolizer of morphine, it could lead to an overdose in the nursing baby and cause very serious side effects. This medicine may cause sleep-related breathing problems (eg, sleep apnea, sleep-related hypoxemia). Your doctor may decrease your dose if you have sleep apnea while using this medicine.
